North Country HealthCare accepts private insurance, AHCCCS, Medicare and offers a sliding fee scale for financially eligible persons. We also accept most major credit cards for payment.
Sliding Fee Scale
North Country provides quality health care to people regardless of their ability to pay. If you are uninsured and financially eligible, our sliding fee scale allows you to pay variable rates based on one's household income and family size (see scale below) that can be as low as 10% of your charges, with a $25 minimum charge per visit and/or payment on your balance at each visit. All sliding fee scale patients are required to supply income verification for the last 30 days (a recent paycheck stub or, if self employed, a copy of last year’s tax return), identification, dependent/household member's information (proof of physical address, a utility bill for example) and your AHCCCS denial letter.

Private Health Insurance
Please check with our front office staff to determine if your health insurance is one of the plans we accept. Please notify us of any secondary insurance coverage. Any insurance co-pay amounts should be made at the time of your visit. You will be responsible for your bill, including the deductible or any other amount your insurance carrier does not pay.
AHCCCS
North Country accepts Healthchoice, APIPA, and Mercy Care Plans offered by AHCCCS. Patients not already on AHCCCS who appear eligible need to apply with our AHCCCS eligibility department. If you are accepted by the AHCCCS program they can provide more comprehensive healthcare and cover more of your medical expenses than the Sliding Fee Scale Program. If you are denied medical coverage, bring a copy of the denial letter to verify that you applied, so we can continue to serve you on a sliding fee basis.
Kids Care Program
KidsCare is a state sponsored health insurance program for uninsured children under the age of 19. North Country is a KidsCare provider through APIPA or Mercy Care. If your children are eligible, they will be referred to KidsCare.
Payment Arrangements
If you are concerned about paying your bill, please talk to someone in our billing department who will work out a convenient payment plan for you. Unless prior arrangements are made, payments are expected on the day the service is performed. You may pay by cash, personal check, money
order, MasterCard, Visa, American Express, or Discover.
Medicare
North Country always accepts new Medicare patients. |
