Primary Care Services

North Country serves as the “medical home” for some 30,000 patients across Northern Arizona providing approximately 75,000 patient visits in 2007.

North Country is a freestanding delivery model through which comprehensive, preventive primary care, dental care, outreach programs, and training/education services are offered across the life cycle of all people's of Northern Arizona including:

• Diagnosis and treatment of acute and chronic illnesses.
• Physical/sports exams to determine risk and provide early detection and treatment of conditions.
• Well child visits and immunizations (including EPSDT examinations)
• Reproductive health, prenatal, and pediatric care services.
• Integrative medicine (acupuncture, acupressure and herbal remedies).
• Dental services including preventive and restorative care ( adult and pediatric).
• Mental health services through an integrated service model.
• Pharmacy services are available on site.
• On-site radiology services and contracted laboratory services.
• Twenty–four hour call and hospital coverage.
• Transportation support

Outreach Programs/Services

Well Woman HealthCheck Program, Health Start Program, Prenatal & Pediatric Care, Maternal Child Health Program, Diabetes Education & Self-Management Program, HIV/AIDS Program, NACASA and RESEP- Radiation Exposure Screening & Education Program ("Downwinders").
